Referral Guidance for Chronic Pain Assessment

A formal written referral is preferred to ensure comprehensive care and appropriate documentation. However, if this is not possible, we kindly ask that the following minimum
clinical information be provided at the time of consultation or referral discussion:

Essential Information to Bring or Communicate:
• Patient details: Full name, date of birth, NHS/hospital number, address, email
• GP details.
• Pain history: Duration, location, nature of pain, impact on daily life
• Medical background: Relevant diagnoses, surgeries, psychiatric history
• Medications: Current and previously trialed analgesics/adjuvants
• Investigations: Key imaging or diagnostic results (if available)
• Previous treatments: Physiotherapy, psychological input, rheumatology, orthopaedics, neurology, procedures
• Referral rationale: Specific concerns, goals, or questions to address
